Child Eligibility Application Checklist

  • Child’s Official Birth Certificate
  • Family’s Income Documentation (full month of pay stubs or other income verification)
  • Residency (i.e. utility bill, rental or lease agreement showing your living address)
  • Family Size (copy of birth certificates for all children living in the home)
  • Child’s Official Immunization Record (Age-appropriate vaccines must be current, we are following the state requirements as listed on www.shotsforschool.org)
  • Child’s Current Physical Exam Record (Including Tuberculosis Assessment and a Blood Lead Test)
  • Child’s Dental Exam Record (Preschool age children must have a dental exam within 12 months of their application)

  • Verification of Pregnancy (Documentation from your doctor/midwife confirming your pregnancy and due date)
  • Family’s Income Documentation (One full month of income verification)
  • Prenatal Exam Record (Copy of your most recent prenatal exam)
  • Dental Exam Record (Copy of your most recent dental exam)
